Chaperone Information

Thank you so much for volunteering to accompany us on our three day adventure of learning and fun! Below you will find all relevant information and links for chaperones.

All chaperones must complete the following by April 1st.

Pay $250 (checks made payable to M.E.S.M.S.)
Fill out and pass a CORI check
Get fingerprinted at one of the local stations

FINGERPRINT INFORMATION

(If you have already been fingerprinted, please send a copy of your FID card to JoElyn Bookman in the School Department's Main Office.)

Click on SCHEDULE A NEW APPOINTMENT
Choose PK-12 EDUCATION (ESE) from the drop-down menu.
Click on ALL OTHER PERSONNEL
Click on YES to confirm agency
Enter the following PROVIDER ID in the box: 02780000

CORI INFORMATION

Please click on the link to the right and print out the CORI form. It should be handed in, or delivered to JoElyn Bookman a the School Department's Main Office.

MEDICAL INFORMATION

All chaperones must complete a medical information form and send it in with their child along with a copy of their insurance card. Click on the link to the right to print out a copy.

PAYMENT INFORMATION

Please make all checks payable to M.E.S.M.S. Feel free to send in installments of $50, but we would like to have all payments complete by April 1st.
