Insurance Coverage
Upon request, you will be provided with a receipt at the end of each follow-up session. While insurance coverage is not guaranteed, the information necessary to submit an insurance claim will be made available to you.
Cancellation Policy
Follow-up sessions which are canceled with less than 24 hours notification may be subject to a $20.00 cancellation fee.
Clients who fail to email/call with a cancellation request, and are a “no show” for their session may be subject to a $40.00 “no show” fee.
If a follow-up appointment is missed, it is up to the client to reschedule promptly. The practitioner will only give 2 reminders to schedule each individual follow-up/chart review.
If a follow-up is lapsed, the client may be subject to an additional fee to compensate for the additional time required to review material that may have been forgotten and to review additional charting. ($15 per each additional 30min after 90min has elapsed)
Future follow-up sessions will not be scheduled until delinquent payments or late fees are collected.
While highly recommended and strongly encouraged, men are not required to attend follow-up sessions and will not be assessed a cancellation or “no show” fee.
Clients Under the Age of 18:
A legal guardian is required to attend the introductory session and all follow-up/chart review appointments. This requirement is for the protection of both the client and the practitioner and to also ensure that the minor client has adequate support at home when learning the Creighton FertilityCare System.
Consent to Instruction in Natural Birth Regulation:
The Creighton Model FertilityCare System will be taught exclusively throughout this program. No instruction in other natural or artificial family planning methods is included.
The Creighton Model FertilityCare System is the only family planning method provided. The method is 95-99% effective when used according to instruction. Like ALL other methods of family planning, this method is not 100% effective.
