New Students must have the following items completed for enrollment.

Send or email a copy of the following Documentation
Copy of child's birth certificate
Copy of child's immunization records
Proof of residency (provide one of the following documents)
utility bill (dated within the past 30 days)
payroll stub (must have current address and dated within the past 30 days)
copy of rental lease and copy of most recent rental payment
copy of title work or closing papers from recorders office
unemployment check (with current address dated within the past 30 days)
welfare/government check (with current address dated within the past 30 days)
$50 Registration Fee - non-refundable and must be paid to reserve a spot in our program
Home Language Survey (see link below)
Race and Ethnicity Form (see link below)
If out of district (you live outside of Shelbyville Central Schools boundaries)- must complete an Out of District Transfer Request (see link below)
For students who are returning from the 2021-2022 school year you will ONLY need to:
