Please mail:
(1) an OFFICIAL first semester/quarter transcript that you order from the college registrar with institution, name and grades,
(2) the form letter, along with
(3) a stamped, self-addressed envelope to:

It is your responsibility to furnish proof of eligibility to the treasurer of the Seward Community Scholarship Committee.

One copy of the form with one transcript is all that is required, even if you have received multiple scholarships. The transcript you submit will not be returned with the scholarship award payment.
