TB and Oral Health Requirement Forms and Information

or confirmation of an appointment required at time of registration for: 

• all TK & Kindergarten students

• all students who are new to the district and are transferring to our district from outside of Santa Clara County, state or country

to be completed and signed by your child's pediatrician.  

The requirement can also be met by providing a valid TB skin or blood test which was completed up to 12 months before registration.

If your child completed the TB requirement in TK, there is no need to complete it again for Kindergarten

required and ok to submit after registration (by May 31) for

• all TK & Kindergarten students

• 1st graders who are new to the district

to be completed by your TK or Kinder's dentist at your child's next dental appointment or latest by May 31st of your child's TK or Kindergarten year.  

For new 1st graders, to be completed at the next dental appointment or latest by May 31st of 1st grade. 

An exam which was done up to 12 months before school entry is acceptable. 

If your child completed the Oral Health requirement in TK, there is no need to complete it again in Kindergarten.
