As a requirement of the Consolidated Appropriations Act of 2021/No Surprises Act, SIHO is providing the required Machine-Readable File (MRF) link available on the SIHO website as of 3/1/25. To comply with the regulation, Self-Funded groups are required to have a publicly available MRF link on their website as well.

Click the arrow in the upper-right corner of the Instructions of the Workers' Comp Form located to the left of these instructions.
Next download the form in order to type directly onto the Workers' Comp Form.
A nurse must complete the form.
The nurse must return the form to Maria Murphy at murphym@scsc.k12.in.us.
How to Print Form

If you have a name change you must present your new social security card to Maria Murphy at the administration building for accounts to be changed to the new name.
