For your child to participate in the activities of the Suwannee Middle School Esports Club which includes console and/or PC video game play, we require your consent for your child to participate. Every scholar gamer’s parent or legal guardian must read, sign, and submit this waiver to the Suwannee Middle School prior to the commencement of game play. Your child must attend and be of good academic standing (at least a 2.0 GPA) with the Suwannee Middle School.

I am the parent or legal guardian of (Scholar Gamer) who is under 18 years of age. I hereby consent to this scholar gamer’s attendance and participation without restriction in the Suwannee Middle School Esports Club consisting primarily of console/PC video game play, video game tournaments and or video game exhibition matches. I understand that the video game (i.e Rocket League) has a rating of “E” for Everyone assigned by the Entertainment Software Rating Board (ESRB). I acknowledge that the Scholar Gamer may choose to participate in non-video game club activities such as commentating and or activities as assigned by the Club Coach(es). In addition, I consent to no claims – past, present, future – against Suwannee School District of event for injuries or loss/damage of personal property during event and or practices. By signing below, I certify that I am the legal guardian of the Scholar Gamer for whom I am signing. I have read and agreed to all material stated above. I have read the Suwannee Middle School Esports Club rules and regulations as provided by the Club Coach(es).
