Includes all over-the-counter medications. To help students treat minor illnesses and injuries, a standing order has been obtained. This means that a school physician wrote an order for school students to receive selected medications within the following guidelines.
To comply with the SASD medication policy, written parental permission is required.
This form must be signed and returned to the school. Please note this authorization is valid for the current school year and MUST be completed each year.

Medication, including over the counter medications will be administered only upon written order from a physician and in accordance with the SASD medication policy.
A "Physician's Authorization for Medication During School Hours" form must be completed by the physician and signed by the parent/guardian before medication can be given in school. In the absence of this form, there must be a written order from the physician and a note from the parent/guardian requesting administration of the medication.
Medication must be in an original, labeled container (from the physician or pharmacy).
Parents/Guardians are required to bring all medication to school.
Medications are kept in a designated, locked area in the nurse's office.
