Preparing Students for Life after High School
About Transition Planning
Transition planning is for all students with an IEP in New York State starting no later than age 15 and means a group of activities that help students with disabilities get ready for life after high school. These activities help students do better in school and in everyday life. The goal is to prepare them for what comes next, like getting a job, continuing their education, or living as independently as possible in their community. On this website, you’ll find helpful resources to guide you or your child as they think about what they want to do after high school and what steps they need to take to reach their goals.
