REMINDERS


  • Please submit your updated Health Form annually in PowerSchool.

  • If your child requires prescription medications (including epi-pens, inhalers. and insulin) to be taken at school, a permission form must be completed and signed by the prescribing physician. Medications should be brought in by the parent/guardian and NOT the student, with the exception of Epi-Pens, Inhalers, and Insulin which may be carried by the student. All medications must be in a labeled container with the student's name. Refer to Medication Procedure

  • Keep your child home when ill: If a fever is > 100, students should stay home until they are fever-free for 24 hours (without fever reducing medication); if the student is experiencing vomiting and/or diarrhea; or if directed by a medical provider to stay home.

  • Health Requirements for entry to school include State-Required Immunizations and Physical Examination per Conway School Board Policy JFAA
