If your child has a life-threatening allergy to food, insect stings, or another threat (such as medication or latex), you are already a pro at planning for their safety!  Providing me with the information that I need (an allergy action plan, emergency medication, a release to communicate with your child's health care provider, & your experience/perspective on what works best for keeping your child safe) to write a plan will help us confidently manage this need at school.

Let's make a plan to manage your child's allergy!

Step ONE: Please contact me by phone at 603-436-2231 ext. 1003 or email sgagne@sau52.org to discuss your child's needs.

Step TWO: Print & complete a Release of Information (or request one to be mailed/sent home) to give permission to communicate and coordinate with your child's health care provider (I can fax required forms directly to your child's health care provider from my office and you can sign it when you bring medication).

Step THREE: Print the preferred district Allergy Action Plan and take it to your child's healthcare provider. The healthcare provider should complete the form and you, the parent/ guardian, should sign it prior to returning it to the school nurse.

Step FOUR: If your child requires changes or substitutes to the food items served in the Dondero kitchen in order to avoid a food allergen, print the Special Dietary Medical Statement and take it to your child's health care provider.

Step FIVE: Bring a supply of medication to me at school. Prescription medications must be in a pharmacy labeled container the prescription label intact. Over-the-Counter medications must be unopened and in the original packaging.
