**BE SURE TO SCROLL TO BOTTOM TO ENSURE YOU FIND ALL THE INFORMATION YOU MAY NEED**
All Students entering HBHS will have a complete physical exam with updated immuniza tions within the past 12 months prior to the start of school. This needs to be updated every two years thereafter; normally in grades nine and eleven. Documentation from the medical provider will be kept in the student's private health record.
According to NHIAA Each year, any student participating in athletics is required to have proof of a physical and current vaccinations dated within 12 months prior to tryouts. This medical documentation should be given to the School Nurse or Athletic Office (You may upload it during registration prior to the start of the school year, mail, attach to email or deliver in person)
NH State Law requires that all students have a completed immunization record on file in the health office before they may begin school. This requirement may be waived by obtaining a religious exemption that will be kept in the student's health record. You may access a list of current immunization requirements by selecting the link below or by consulting your medical provider.
