Request your Report Card (Form 138), certificates (ESC or Ranking/Honors), and principal's recommendation.

Request a copy of your Good Moral Certificate (GMC) and other pertinent documents. Select the level that will correspond to your request.

Send your proof of payment (school fees) for transactions made this Academic Year 2021-2022.
