Certificate of Insurance

For COIs, students should contact the school's Gear Manager. When requesting a COI, There is a 72 hour turnaround time for requests, not including weekends.

The following information is needed for every COI request:

• Shoot Date

• Location Name and Address

• Location contact person with email and phone #

• Any specific requirements

Requests should be made using this form: https://forms.gle/L256RSpHXzdUkTet8 

The gear manager will relay this information to the insurance company and will send the COI to the student as soon as it is received.

Note: When shooting on a private property, Students should assume that the property owner will require Proof of Insurance.
