A Registered Nurse (RN) is assigned to every school and is the first point of contact for your questions and concerns. They have a Baccalaureate degree or above, have additional post-graduate education related to school health issues, and have a School Nurse Health Services Credential through the California Commission on Teacher Credentialing.
School nurses are the primary health care providers within the educational environment promoting optimum health status, physical, mental and emotional health, so all students may achieve their fullest potential as a learner and a person.
An IHP is a plan of care written by the registered nurse for students with or at risk for physical or mental health needs. It is the responsibility of the school nurse to annually evaluate the IHP, as well as to update the plan if deemed appropriate, to reflect changes in the student’s healthcare needs and address nursing interventions and/or student healthcare outcomes.
School nurses create an IHP for select students with healthcare needs that, if not addressed, may negatively affect, or have the potential to affect, attendance and/or academic performance. These students may have chronic health issues or have an acute alteration in their health status that may temporarily require specialized nursing care. Priority for IHP development must be given to those students who require significant health services at school, have a medical diagnosis that may result in a health crisis, and/or students with health conditions addressed in a Section 504 Accommodation Plan or an Individualized Educational Program.

The mission of the Home/Hospital Program and Transition Supports School is to provide students with seamless instructional services while supporting their academic, social/emotional, and transition needs during a time of crisis.
We serve students in grades K-12 whose medical condition prohibits their attendance in a regular school or who have a short term placement in a therapeutic setting. The majority of our students are enrolled for a short period of time before transitioning back to their regular school, while some attend for a longer period of time.
