Please use one of the following forms when requesting a mini-grant, processing your reimbursement, or when you need a tax exempt form from Harvest PTO. You will use these forms if turning in a PTO-related expense either from a volunteer, committee coordinator, or staff member.

PTO REIMBURSEMENT REQUESTS:

All reimbursements should be requested NO MORE than 30 days from the date of purchase.

TAX EXEMPT FORM:
Please remember to use your tax exempt form when shopping to take advantage of Harvest PTO tax exemption. In MOST cases, tax will not be reimbursed.
