Work Permit

Any student who is under 18 years old will need a completed work permit when they get a job. 

AFTER you are hired

Fill out the top part of the work permit form-see attached-and have a parent sign it

Have your employer complete the work permit form-see attached

Have your physician complete the physical form-see attached 

If you have a physical on file in the Athletic office, you do NOT need to have your doctor complete this portion.

You can get a copy of it from the Athletic Office and attach it to the application.

Turn in the completed forms to Mrs. Altenhof in the counseling office.

She will email you when the permit is ready for you to pick up.

Please allow 1-3 business days for her to process the permit online.

She will return it to you to sign and to give to your employer so you may begin working.
