Parental Consent for School Health-Related Services
San Angelo ISD is required by law (Senate Bill 12) to obtain parent/guardian permission before providing health-related services to students. Please use this form to indicate your consent (or refusal) for your child(ren) to receive health-related services at school during the 2025-2026 school year.
