NEW CHANGES: FHSAA REQUIRED Forms for the 2026-2027 School Year
REQUIRED PAPERWORK FOR INTERSCHOLASTIC ATHLETIC PARTICIPATION FOR ALL STUDENT-ATHLETES
(Required prior to ANY athletic participation including optional summer workouts & conditioning)

*NEW CHANGES* Beginning July 1,2026 the state of Florida has mandated heart screenings for any sudent-athletes new to FHSAA sanctioned sports. CLICK HERE FOR MORE INFORMATION
Current EL3 Form
The EL2 & EL3 form can be found in the Magnus Health Portal on SSES online under the Resource Page. Please see the Magnus Parent Letter for more information.
All student-athletes must complete the NFHS Sportsmanship course to be eligible for participation. Once completed all certificates of completion should be uploaded into the Magnus portal.
To complete the course, please follow the directions below.
Navigate to the NFHSLearn Sportsmanship course.
You must have a free NFHS account. So please sign in or create an account.
Select the state of Florida as the state.
Click on "Start Learning".
Complete the course.
Once the course is complete and you receive our certificate of completion, upload the certificate to the Magnus portal.
NEW SAINT STEPHEN'S STUDENTS ADDITIONAL FORMS
If the last grade the student attended was not a terminating grade at that school (i.e. this student could have continued to attend the previous school), the student will be required to have a GA4 Form.
NEW SAINT STEPHEN’S INTERNATIONAL STUDENTS ADDITIONAL FORMS
The Florida High School Athletic Association (FHSAA), our governing body, requires an application for athletic eligibility to be submitted. Please contact the athletic department IMMEDIATELY to begin the process.

Signed & Completed copy of I-20 Form (F-1 students) or DS-2019 Form (J-1 students)
Original Language Transcripts since the 8th grade (or previous 2 consecutive semesters, if younger than 8th grade)
Officially translated copies of transcripts since the 8th grade (or previous 2 consecutive semesters, if younger than 8th grade)
