Sabine ISD provides a supplemental insurance policy through National Health Insurance Company for student-athletes. Any injury sustained during a school-sponsored athletic activity is eligible to use the supplemental insurance policy.
Your insurance will act as primary and any remaining balance will be handled by the secondary insurance provided by the school district.
This form will be completed and given to the student-athlete prior to any scheduled doctors visits.
