Needs an asthma inhaler or nebulizer at school
Has a food or environmental allergy and needs an Epi Pen at school
Will need medication given to them during the school day
Has a chronic health condition
Has any other health concerns
Your child may need a Health Plan
If you have questions about your child's health needs, please contact the school nurse
Your child's healthcare provider fills out the Prescription Medication Authorization Form (located below)
Prescription medication is given to the School Nurse in its original package from the pharmacy
It must have your child's pharmacy label on it
Example: ADHD Medication to be given at School
The Over-the-Counter Form (located below) is filled out by the parent/guardian and
The medication is brought to the Health Office in a brand-new and sealed package
Example: Tylenol for headaches, Eczema cream, sunscreen, Lactaid
