This link can be used to change any contact information, such as phone, email, or address. Please click on link and follow the directions
This link is for students that live outside of the CHS boundaries, but would still like to attend our school. This form needs to be submitted every year for approval by District Office. Please click on link and follow the directions. **Make sure to apply for the correct school year when applying**
If you AND your student live in a residence with a friend or family member, you and the property owner will need to fill out an Address Verification Form and get it notarized. You can then turn both that and the owner's proof of residency to us at the front office or through the above Document Upload Link. This is something that has to be renewed each year.
