Your student may need a Health Plan with the school nurse If they:

Will need medication given during the school day

Your child's healthcare provider fills out the Prescription Medication Authorization Form and
Prescription medication has to be given to the School Nurse in its original package from the pharmacy.
It must have the pharmacy label on it.
*No prescription medications can be given to your student without a prescription order from their doctor*

Over-the counter (non-prescription) medication must be supplied to the Health Office by the parent/guardian in the original, sealed container. This medication will only be given if a NM Department of Health (NMDOH) Standing Order is available. If not provided by the NMDOH, we must have an Authorization to Administer Prescription Medication form. If over-the-counter medication is needed for more than 3 consecutive days, a written order from a health care provider is required.
