Will need medication given during the school day

Have any other health concerns
No prescription medications can be given to your student without a prescription order from their doctor
Your child's healthcare provider fills out the Prescription Medication Authorization Form and
Prescription medication has to be given to the School Nurse in its original package from the pharmacy.
It must have the pharmacy label on it.
The Over-the-Counter Form is filled out by the parent/guardian and
The medication is brought to the Health Office in a brand-new and sealed package
Over-the-Counter medication (OTC) form
may be requested from the Health Office.
