Upcoming 2024/2025 School Year
Please make sure your child has all the required immunizations or completed immunization exemption form turned in before the start of the school year. If they do not have the required immunizations or paperwork, your student will not be able to start on the first day of school.
If you need health paperwork (such as forms to give to your child's medical provider for Asthma, EpiPen, Seizures, Diabetes, or other health concerns), please visit the Health Plan and Medication Forms Page.
