Prescription medications can be given to your child in the Health Office if:

• Your child's healthcare provider fills out the prescription medication authorization form

• Prescription medication is given to the school nurse in its original package from the pharmacy

• It must have your child's pharmacy label on it

Students are not permitted to carry any medications on their person unless the required paperwork has been given to the school nurse

OTC medications can be given in the Health office as long as it is covered under a NMDOH standing order, and  with an OTC Medication Authorization Form completed by a parent/guardian. The Health Office does not supply medication. It must be provided by a parent/guardian.
