Upload your child's immunization form during registration. If your child requires any immunization updates, we will contact you. You can see the updates to immunizations on the immunization tab.

During registration, please list all of your child's health conditions and/or medications (ex. specific allergies, asthma, constipation, eczema, uses inhaler, etc.).

Please remember to schedule doctors appointments prior to the start of the school year for your child's doctor to fill out all necessary paperwork. Please go to the Medical Forms page to find a link to the forms you need.
