2024/2025 PHYSICAL SUBMISSION AND CLEARANCE INSTRUCTIONS

PLEASE READ ALL INFORMATION BEFORE SUBMITTING

Welcome, and thank you for your interest in Rio Rancho Public Schools Athletics! Rio Rancho Public Schools value the important role that athletics plays in contributing to the overall high school experience. 

Please be aware that you must have ALL required documents in order to submit them to the clearance queue at your respective school.  The packet should be uploaded in ONE compete submission; do not submit in multiples. Once received, your school's athletic department will review them for accuracy.  Provided all documents are complete, your student will be cleared, and you will be notified via the email address you’ve provided during submission.  If cleared, your student will be placed on the 23/24 Athletic Clearance list at his or her respective school and you will need to do nothing further for the 23/24 school year.  If your documents are not complete or there is a question regarding the submitted documents,  you will receive an email via the email address you provided during submission requesting resubmission. 

A couple of things to remember as you start the clearance process: 

1. PLAN AHEAD- TYPICALLY CLEARANCE WILL NOT TAKE PLACE ON THE SAME DAY.  GIVE YOURSELF AT LEAST A WEEK PRIOR TO YOUR STUDENTS START DATE. IT COULD TAKE 3-5 DAYS TO PROCESS

2.  8TH-GRADE PARTICIPANTS MUST SUBMIT THE PACKET TO CHS/RRHS VS. THEIR RESPECTIVE MIDDLE SCHOOL IF THEY WISH TO BE CLEARED BY CHS FOR PARTICIPATION AT THE HIGH SCHOOL LEVEL. 

REQUIRED ITEMS FOR PHYSICAL PACKET

SAMPLE OF WHAT YOUR PACKET SHOULD CONTAIN WHEN SUBMITTING.  

Before a student is allowed to participate in any RRPS Summer Athletics, each student will need to have all paperwork turned in to the appropriate High School Athletic office, on file, and cleared for participation by that High Schools athletic department.  

Please read through the requirements below carefully as some requirements/forms have changed as well as the method of submission. 

24/25 CLEARANCE START HERE 

PRIOR TO SUBMITTING A PHYSICAL PACKET, your student must be registered/pre-registered for the school in which you wish for them to be cleared for participation. Click  HERE to complete this process.  

You will need the following documents below to complete the physical packet

1. Click HERE for NMAA Physical Packet   Physical Exam (YOU MUST USE THIS EXACT DOCUMENT) must be conducted, and the form must be dated on or after April 1, 2023, to be valid for participation in the summer of 2023 and the  2023/24 SY.  The linked document is a pdf. print this document and take it with you to your physical appointment with your physician (please note that not all of these documents will be returned to the school for clearance; please reference the sample packet linked above). Do not open the document in a Google Doc and try and print or type into the document as it will not stay formatted. 

2.  Click HERE for NFHS Student Concussion Course sign up for an account, add the student concussion course to your basket (the course is free), and complete the course. Please be sure when creating the account that it is in the student's name so the certificate will be generated with the correct name.  When you register for an account, the school you choose will not matter as long as you choose NM as your state. Also, be sure that the course is the CONCUSSION FOR STUDENTS COURSE.  Any other course will not meet the state law requirement.  At the end of the course, you will be issued a certificate of completion.  This certificate will need to be downloaded and submitted via the portal along in with your other documents.   Must be dated on or after April 1, 2023. 

3. Copy of the front and back of your health insurance card  (click HERE if you do not have health insurance and need to purchase the school offered health insurance) 

Processing time will vary depending on how full the portal is and other prioritized duties of the Athletic Office.  

 Do not plan for a same-day clearance.  Also, keep in mind that packets that must be resubmitted will be placed at the end of the line and processed in their own order received. 

Homeschool and Charter School students must register with CHS as an "Athletic Only" student in order to participate in our Athletic programs:  - click HERE to complete this process. 

Transfer students click HERE to access the Transfer Student checklist. Please complete this form gather the documents listed on this sheet and email them to chsphysicals@rrps.net 

You are a transfer student if you: 

• participated in athletics as an 8th-grade student at the high school level that was not Cleveland HS.

• participated in athletics at the high school level in grades 9-12 at a school that was not Cleveland HS.

READY TO UPLOAD...

• Please only submit once.  

• You do not need to submit a packet for each sport. 

• DO NOT EMAIL YOUR PACKET IN; all packets must be submitted via the portal link below. 

• Your packet will initially be routed to Athletic Office for review.  In an event your packet is received incomplete, you will receive an email stating such and you will be required to resubmit all documents to be reviewed again. 

•  If your packet is submitted and is correct,  you will receive an email stating your athlete is cleared. 

• Packets are processed in the order received.  If your packet is submitted incomplete, your resubmission is not given priority and will be am-processed in its own order received.  Processing time will vary depending on how full the portal is and other prioritized duties of the Athletic Office. DO NOT PLAN FOR A SAME-DAY CLEARANCE.

THE 23/24 PACKET ONLY CONSISTS OF 5 PAGES TOTAL.
