Process to Apply:
Complete this Google Form. This form will be shared with you during your annual ARD or 504 meeting.
The Campus Testing Coordinator will email you a parent consent form to complete via docusign (Stony Point Testing via Adobe Sign)
Once you submit the consent form, the Campus Testing Coordinator will apply to The College Board on your behalf for testing accommodations.
Decisions can take 7 weeks to process, please allow plenty of time before testing.
Accommodation approvals are valid for up to five years through College Board. You do not need to re-apply each year!
Deadlines for Accommodation Requests
If the deadline has passed, we can still apply for accommodations, it is just not guaranteed to have a decision before the test date.

Waiving Your College Board Accommodations
There may be a time where you would like to waive specific accommodations for a test administration. In order to do so, College Board requires a signed letter from a parent/guardian waiving the accommodation. SPHS must turn in the letter with the students' test materials in order for the materials to be scored. Please email the signed letter to the SPHS testing coordinator.
The letter must include the following information:
Parent/Guardian Name + Student's Name
Student's AP ID number (AP exam only)
Student's College Board SSD number
Statement indicating which accommodation(s) you would like to waive
The test and date of the test that you would like to have the accommodation(s) waived
Parent signature
You can use this template to submit to the SPHS testing coordinator.
