Medications at School

Students that will need to take prescription medication while at school are required to have a request to administer medication form signed by a physician:

***ALL MEDICATIONS must be brought to the nurse in person. Please do not send any medications with your student or in their backpack.

Allergy/Anaphylaxis 

Students with severe life threatening allergies (anaphylaxis) requiring an EpiPen at school will need the following form(s) signed by a physician :
