Hour Submission Forms

Important

1. PLEASE DO NOT SUBMIT YOUR HOURS TWICE! If you made a mistake on your form submission, please email us, regarding the mistake, do not submit it again. 

2. You may submit previous hours as far back as May 2025 for 2025-2026 school year. 

3. To complete the hour submission form, you must be signed into your student email.

4. We will not accept any hours that aren't verified by the sponsor of the organization that you have volunteered at. A verification means a signature from your sponsor at the bottom of your hours sheet. If you have any extenuating circumstances, please email us at the Green Cord email, and we will see how you may proceed. 

5. Any pictures you submit of your hours sheet MUST SHOW THE COMPLETE SHEET or it will be considered invalid.

6. You must use a different google form for each organization you serve. In one google form, you can attach up to 10 paper forms that record the volunteering hours from the same organization.

7. The Green Cord sponsor and officers will review and update your hours. If there is a question about your hours after they are reviewed, the officers will contact you. If you do not respond in a timely manner, those hours will be forfeited. Remember that students are responsible to clear up questions and check their hours, not parents.
