BIRTHDAY BOOK CLUB FORM
Celebrate your child on their birthday by donating a book to the Chandler Oaks Library. Once the $20.00 payment is received, your child will select their book from the new and popular titles located in the library.
A bookplate with the honoree’s name and birthdate will be placed in the front of the book, and they will be the first to check out the book. The honoree’s name will also be added to our Birthday Book display in the main hallway.
Here’s how it works:
Print and fill out the Birthday Book Club form at the bottom of this post or pick up a form from the library. Please follow the directions on the form to pay online ($20.00 + $1.50 transaction fee). *For families with multiple students, you are able to purchase more than one student's birthday book, in one online transaction, and save on the extra transaction fee. (each child will need a form)
A few weeks before your child’s birthday, I will send a reminder to the classroom for them to come by the library to select a book.
The book will be presented to your child on or near their birthday and they will be the first student to check out their special book.
We can celebrate your child’s birthday no matter when it is– don’t worry if it has already passed! Details for summer birthdays are noted below and on the form.
Please note: the program ends on Thursday, April 30, 2026 for this current school year, so that we may ensure your child can check out their book before the end of the school year!

SUMMER BIRTHDAY?
The dedication will be on the student's half birthday.
June will be in January.
July will be in February
May birthdays will be dedicated at the beginning of May.
