The Office of Student Financial Services will issue a signed letter verifying your eligibility for financial aid to provide to a leasing agent/housing complex, external scholarship organization, etc. This is not a form for students to complete to see how much financial aid they will be eligible for.
Your FAFSA was flagged for unusual enrollment history review by the U.S. Department of Education because you received financial aid at multiple education intuitions during the past four academic years. This flag requires the Rosalind Franklin University of Medicine and Science to review your enrollment history to determine your eligibility to receive federal student aid.
This form is for students who wish to return loans that have already been disbursed to their student account.

Students interested in Federal Work-Study (FWS) should submit the Federal Work-Study Interest Form and the Office of Student Financial Services will determine eligibility.
Students enrolled in an online program who wish to have financial aid cover the costs of the program must fill out this form.

The Office of Student Financial Services does not guarantee approval and/or 100% reimbursement of professional judgment budget requests. Adjustment is reviewed on a case-by-case basis. This is a reimbursement process through the Graduate PLUS Loan.
