How to Submit Forms?

Option 1: Completed applications must be submitted to Martin Luther King High School. Parent(s) or Guardian(s) may deliver the applications in person to the A-Building (Maria Ortega). She will date stamp all the forms and submit them to Ms. Guzman PLTW Coordinator. The office is open from 8:30 AM to 4:00 PM. All completed applications must be submitted before Friday, December 12, 2025, at 4:00 PM in person.

Option 2: Completed applications may be emailed as a PDF File to nguzman@riversideunified.org before the deadline of Friday, December 12, 2025, at 5:00 PM.

Important: This form is to be attached to the Inter-District Transfer Release once it is signed off by the student’s

district of residence. Please note that this transfer request is incomplete until both this form and the Inter-District

transfer release forms are submitted. Incomplete forms will not be processed.
