In addition to submitting data every time you test with students (fall and/or spring), please complete the following reporting documents linked below:

1. Annual Application (beginning of school year)*
2. Stipend Report (end of school year)*
3. Student Post-Survey (end of school year)*
4. End of Year Teacher Survey (end of school year)*
5. Photos (end of school year)

*Indicates mandatory reporting. Photos are optional, though always welcome!
