The client is financially responsible for the services provided. Financial arrangements must be made before admission. Please ask your primary clinical staff or student for more information should you have questions.
WEvolve utilizes fiscal management in accordance with Generally Accepted Accounting Principles. WEvolve and its affiliates bill the client’s insurance as a courtesy. If the client has insurance and refuses to have insurance billed by WEvolve, for the client, the financially responsible party will be charged the insurance rate and given a Super Bill to submit to your insurance company.
WEvolve will offer a Sliding Fee Discount Program to all who are unable to pay for their services. This program is designed to provide discounted care to those who have no means, or limited means, to pay for their services (uninsured or underinsured). WEvolve will base program eligibility on a person’s ability to pay and will not discriminate based on age, gender, race, sexual orientation, gender identify, creed, religion, disability, or national origin. The Federal Poverty Guidelines (http://aspe.hhs.gov/poverty) are used in creating and annually updating the sliding fee schedule (SFS) to determine eligibility based on family size and income. No one will be denied access to services due to their inability to pay. Please ask your primary clinical staff or student about the Client Discount Program and associated application.
