Our Harm Reduction initiatives offer clean needle access (in exchange for used ones), drug checking, safer supplies, STD prevention, HIV/HCV and Syphilis testing, and distribution of Naloxone. We provide both online and in-person overdose prevention education. Additionally, our Harm Reduction Program facilitates connections to Medication Assisted Treatment (MAT), encompassing Suboxone and Methadone treatment, counseling services, and case management support.
Our unhoused outreach services are designed to address the diverse needs of adults experiencing homelessness by integrating mental health, medical, substance use, and support services. Our dedicated team consists of professionals with a wide range of expertise, including mental health and substance use practitioners, registered and licensed practical nurses, certified peer specialists, vocational specialists, case managers, clinical social workers, and psychiatric providers. Our approach focuses on providing a seamless continuum of care, guiding adults experiencing homelessness through the journey from the streets to permanent housing while ensuring a vital connection to ongoing, tailored mental health services to support sustained recovery.
Our linkages program engages multidisciplinary teams and involves both individuals and their communities. Leveraging Comprehensive Community Support Services (CCSS), we aim to empower, facilitate recovery, and enhance the overall well-being and integration of those we serve, ultimately fostering independence and reducing stigma associated with mental health and substance use issues. CCSS refers to a multifaceted and person-centered array of integrated services within the realm of mental health and substance use care, intended to assist individuals facing these challenges within their local communities. CCSS encompasses case management to coordinate various resources, crisis intervention for immediate support during critical situations, skill-building, therapy, medication management, peer support, housing assistance, access to social services, education, and advocacy.
Our certified peer support workers meet individuals in the field and have personal experience with specific challenges or conditions. We provide empathetic assistance, empowerment, emotional support, education, role modeling, advocacy, hope, motivation, and skill-building to others facing similar experiences. This approach is based on the understanding that peers, with their firsthand knowledge, can uniquely relate to and support individuals in their recovery or management of conditions, reducing stigma, fostering a sense of community, and complementing professional services in various fields such as mental health, addiction recovery, and chronic illness management.
Substance use counseling is a specialized form of counseling aimed at helping individuals grappling with problematic substance use or behavioral addiction issues. Substance use counselors assess the severity of addiction, develop personalized treatment plans, and provide individual and group counseling sessions to address the physical, psychological, and social aspects of addiction. They educate clients about substance use, offer crisis intervention, involve family and support systems, and refer clients to additional services when needed. By providing ongoing support and follow-up, substance use counseling aims to help individuals achieve sobriety, enhance their overall well-being, and rebuild their lives after addiction, making it a vital component of addiction treatment and recovery.
Our crisis intervention team offers a short-term, immediate form of psychological and emotional support to individuals facing acute distress or crisis situations. It involves assessment, emotional support, crisis stabilization, problem-solving, safety planning, and, if necessary, referrals to other services. Crisis intervention is offered by trained professionals in various settings to help individuals regain emotional stability, make decisions to address the crisis, and connect them to appropriate resources and ongoing support, with the primary goal of mitigating immediate distress and reducing the risk of harm.
