Kindergarten parents : Please take a look at the Health Form section and have the appropriate forms, explained below, completed for your child by their health care provider (HCP) prior to the start of the school year in September.
Birthdays after October 1st, please have your HCP complete the Health Physical Form based on your child's physical from last year. Please include seasonal allergies on the health physical form if your child experiences symptoms. You must include your child's Immunization Record or Letter of Religious Exemption (signed) with their health physical form.
The New Jersey Department of Health (NJDOH), Vaccine Preventable Disease Program is reminding parents and healthcare providers (HCPs) that all children must comply with age-appropriate immunization requirements set forth in N.J.A.C. 8:57-4. Parents and guardians should plan ahead and schedule appointments with their pediatricians now to avoid the back-to-school rush in August.
Please submit ALL required paperwork prior to school starting. Any students transferring into district from out of state or county, there is a 30 day period for medical documentation to be submitted.
If the paperwork is not submitted withing the first 30 days, then your child may be excluded until the paperwork is turned in.
Children who are not up to date on their immunization requirements and/or do not meet eligibility for provisional admission, will NOT be allowed to attend school until immunizations are received or applicable exemptions are filed.
New Jersey still allows for medical and religious exemptions when applicable.
If your child has Food Allergies and your child has an epinephrine pen, please have the Epinephrine Administration Form (see below) completed by their doctor. On the first day of school, you will need to bring a box with two pens to the nurse's office with the completed forms.
The food at school is provided by Maschios. They have a registered dietician that will be able to go over the menu and ingredients with you and help you create options for your child.
If your child has Asthma, the Asthma Action Plan (see below) needs to be completed by their primary care physician before the start of school. On the first day of school, the parent will need to bring in an inhaler and signed action plan for your child.
If your child requires Medication (prescription or over-the-counter) during the school day, please have the Medication Administration Form (see below) form completed by their doctor.
