Data Collection Surveys

Step 1-3: Assessing Children

This form helps us track the process reliability during the administration of quizzes using the Summer Literacy Coach. This survey is intended to be used during assessment windows only.

Step 4: Plan & Deliver Learning

This form helps us track the teacher's experience as they plan and use the Summer Literacy Coach to inform and influence their instruction. This survey is intended to be used on a weekly-basis as long as teachers are using the Summer Literacy Coach to plan instruction.
