Parent/Guardian Referral for
School Based Counseling
1. Email or meet with Counselor with brief synopsis of concern.
2. Counselor will respond in one of the following ways depending on the information provided:
Reply with more questions.
Observe student in class and provide feedback or plan
Request the teacher or parent complete a referral form to provide more specific details
3. Email to tmatheson@pleasantvalleysd.org or contact the school office with your concern.
A student should complete a request to see the Counselor form located outside of C-1 or tell their teacher or parent they would like to see the Counselor. Parents or teachers can contact the school counselor through email or a phone call. Students may also complete the self-referral form using their pvsd google account.
