Chronic Illness/Homebound

If your child had a Chronic Health Placement or Homebound Placement, for the 2019-2020 school year, be sure to secure the appropriate paperwork from the school nurse for the 2020-2021 school year. This request must be made annually per district policy, it does not renew automatically.
