Purdue Global's medical plan provider is United Health Care Choice Plus - United Medical Resources (UMR).
The Employee Retirement Income Security Act (ERISA), Department of Labor (DOL), Department of Health and Human Services (HHS), and Internal Revenue Service require plan administrators to provide certain information related to their health and welfare benefits plan to plan participants in writing. To satisfy this requirement, please review the compliance notifications included in the compliance bundle. These notices explain your rights and obligations in relation to the health and welfare plan provided by Purdue University Global.
Purdue Global's prescription plan provider is CVS Caremark.
Purdue Global will use CVS Caremark in 2025 for the faculty and staff prescription plans.
Same formulary drug list: The formulary drug list is the same; however, it is standard practice for formulary updates to be made on January 1 each year. Review the 2025 formulary drug list.
CVS member portal online or mobile app: Once you have your new ID number, you can register your online account to take advantage of the many resources CVS Caremark provides.
CVS pricing tool: Purdue Global employees are encouraged to use the Purdue pricing tool page until CVS makes the Purdue Global pricing tool available.
Purdue Global's dental plan provider is Delta Dental.
Dental plans are administered on a calendar year. Elections made during the annual open enrollment period will be in effect from January through December unless you have a qualifying change in family status or loss of eligibility. Premiums are taken from your pay before taxes are calculated. Any deductible or benefit limits associated with your plan are tracked on a January – December basis. All benefits paid by the dental plan contribute toward the plan’s annual maximum benefit.

You may view and enroll in dental benefits during annual open enrollment and as a newly eligible employee. Go to the Purdue OneCampus page, and choose the Benefits Enrollment - Active tile to access enrollment.
Purdue Global provides vision coverage through a program called Vision Service Plan (VSP.
VSP Primary EyeCare Plan: This plan provides supplemental medical eye care services for the detection, treatment, and management of ocular and visual conditions.
The member’s health insurance carrier should be billed as primary when other plan coverage exists and you participate in the health plan’s network. When VSP administers the secondary plan, the member will receive a specified allowance for each service (exam, lenses, frame, or contacts) that will be used to pay up to, but not more than the billed amount.
Free benefit*, but it must be elected during open enrollment.

Vision Care is now separate from Medical. You must have elected Vision during open enrollment to have coverage, unless you qualify under an eligible life event. Contact Purdue Global HR at pghr@purdueglobal.edu for more information.

The simple way to save for out-of-pocket expenses. An FSA is an account that you contribute money into from your paycheck before taxes are taken out. You'll save money when you use it to pay for health and dependent care expenses.
Save money with an HSA. With an HSA, you can reduce your taxable income and increase your take-home pay. Just enroll and you'll start saving money on eligible health care expenses for you, your spouse, and your tax dependents.

What is long-term care?
It’s a term used to describe services people need when they are unable to care for themselves. This can range from help at home with daily activities like meal preparation and housekeeping, to personal care services like bathing and dressing. Long-term care also is provided in assisted living facilities, which are home-like settings that offer safety and security, plus nursing homes that provide round-the-clock skilled nursing care.
What is long-term care planning important?
Long-term care is an important part of a personal financial plan. As we age, the likelihood of needing long-term care increases and the average cost of home care is $43,000 per year or more. Your health insurance, disability and Medicare do not cover most long-term care costs. Long-term care insurance is designed to protect your retirement savings from the cost of long-term care services if you need care in the future.
How does long-term care insurance work?
Long-term care insurance pays benefits for covered long-term care services when your doctor certifies you are unable to perform at least two of the six activities of daily living or you need continual supervision due to a severe cognitive impairment.

Why should I consider long-term care insurance now?
Even though you may be young and healthy, there are several good reasons to consider purchasing a long-term care insurance policy now:
You’re never too young to need long-term care. Accidents and illnesses requiring long-term care services can strike people of any age
It will never be more affordable. The cost for a long-term care insurance policy is based on your age when you buy your policy. That means every year you wait, the cost will go up.
Your ability to qualify for long-term care insurance is based on your current health. If your health changes, you may not be able to purchase long-term care insurance at any price.

Qualifying Life Events
Qualifying life events are significant life changes that enable you to modify your benefits outside the annual open enrollment period. Common examples of these events include marriage, divorce, the birth or adoption of a child, and loss or gain of other coverage.
When such an event occurs, it's crucial to report it within 30 days of the event. Failure to report a qualifying life event within the 30-day window may result in having to wait until the next annual open enrollment period to make changes.
