I tried to create an account and I am getting an error message says I already have an account.

If you cannot remember your username or password, click on Step 2 and choose "Forgot your Login/Password?" If you do not know your email address  or username on file, click the "Still need help" button at the bottom of this page and someone from our office will call to assist you.

I don't have current immunizations but my child is up to date and has always lived in Rhode Island.

If your child has always been vaccinated in Rhode Island, our nurse can search the RI database for current immunizations. 

I am staying with someone else and don't have two proofs of residency in my name at this time.

We require two proofs from the homeowner/primary renter, one proof from you and a notarized letter stating that you are currently living at that address. 

There isn't enough room to upload all of my paperwork, how do I submit additional documents?

Additional documents can be emailed and we will add them to your child's registration (registration@psdri.net).

I need to speak with a Spanish or Portuguese interpreter.
