If a student is chosen to participate on any sport team, cheerleading squad or drill team squad, a MSHSAA Sports physical must be on file prior to participation in the first contest:
Pattonville High School will be going online with all forms needed to compete in MSHSAA sponsored sports and Marching Band. If your son/daughter will be participating in any athletic team tryouts or marching band, where a physical is necessary, you will need to register on Healthy Rosters. Healthy Rosters will be our online site for all forms that were previously included in our participation packet. Physicals, emergency contact, concussion information, insurance information, and all necessary signatures will now be done through Healthy Rosters. The Activities Department. will no longer accept participation packets or paper copies of any information.
Parents of any student who wants to try out for an athletic team at Pattonville High School. This includes activities such as Marching Band, Drill Team, and Cheerleading as well.
You will receive an email from Healthy Rosters asking you to register in the spring and throughout the summer. Once you register your son/daughter will go into the system and all forms will be available online for you to complete. All incoming freshmen will need a physical that is dated on or after May 1st of the year they will enter high school. All other physicals will be valid for two years pending physician approval. Healthy Roster will list an expiration date for all documents, and will notify you when they are set to expire. The physical form can be printed off of Healthy Rosters and taken to your doctor to fill out. Once the doctor has filled out the physical and signed and dated it, you will be able to take a picture of the form and upload it to Healthy Rosters. All other forms can be completed online without printing or uploading copies. Our athletic trainer will then review and accept these documents or request more information. Healthy Rosters will give you an up-to-date checklist of what is needed for participation, and documents needing attention will be indicated. Coaches/sponsors will have access to see if their athletes have completed all forms or not and whether they are allowed to participate or not.
Follow these easy step-by-step instructions for creating your athlete’s profile on your laptop or computer.
On your laptop or desktop computer, locate your invitation email from no- reply@healthyroster.com. It was sent to your email address that is linked with your child’s PowerSchool information.
At the bottom of the email click on the link that says, “Click here to sign up via the website instead”
Enter the code listed in your invitation email when prompted, then follow the on-screen instructions for completing your athlete’s profile. Please fill out all boxes to the best of your ability.
When you are ready to begin checking off documents for your child’s participation packet, click on the “Documents” section to the left of your screen. If you are having trouble with your athlete’s documents, please view our “Document Instructions” link posted on the athletics website.

Follow these easy step-by-step instructions for completing your athlete’s documents.
Log on to your athlete’s Healthy Roster profile via your laptop or desktop computer, or by using the Healthy Roster app.
Locate the “Documents” section on the left side of your screen, or by clicking on your athlete’s profile on the mobile app.
Once in the “Documents” section, you will be able to download and print a copy of the MSHSAA pre-participation physical exam form to take to your physician, as well as view and sign the Pattonville Code of Conduct, concussion information, and fill out your athlete’s insurance information.
The MSHSAA pre-participation physical exam form must be filled out as a hard copy by a physician and uploaded to your athlete’s profile. This can be done via the Healthy Roster app by taking a picture of all pages of the exam with your mobile phone. There is a YouTube video in the MSHSAA physical exam section that gives step-by-step instructions for uploading the document.
Please make sure uploaded images are in-focus and not blurry. Unreadable documents will not be accepted.
Incomplete documents will appear as a red document icon on your athlete’s profile; documents that have been filled out but are missing one or more items will appear as a yellow document icon on your athlete’s profile. All documents must appear green prior to your athlete beginning their sport.
Physical exams have to be reviewed by the Athletic Trainer prior to completion, and will not appear as a completed document until they have done so.
