PSB Guidelines for School Entry and Sports follow Massachusetts immunization requirements.
Documentation must be translated. Immunization dates must include month, day and year to fully establish your student’s immune status.
Preschool (and all higher grades)
DTaP: 4 doses
Polio: 3 doses
Hepatitis B: 3 doses (last dose after age 6 months)
Hib: 1 to 4 doses
MMR: 1 dose (after 1st birthday)
Varicella: 1 dose (after 1st birthday)
Kindergarten (in addition to preschool requirements)
DTaP: 5 doses (last dose after age 4)
Polio: 4 doses (last dose after age 4)
Hepatitis B: 3 doses (last dose after age 6 months)
MMR: 2 doses (first dose on or after 1st birthday)
Varicella: 2 doses (first dose on or after 1st birthday)
Lead test: within the past 12 months
Tuberculosis risk assessment or skin test: within the past 12 months
Vision screening: within the past 12 months
7th Grade (in addition to preschool & kindergarten requirements)
Tdap: 1 booster dose
Meningococcal Conjugate (MenACWY): 1 dose
11th Grade (in addition to preschool, kindergarten, and 7th grade requirements)
Meningococcal Conjugate (MenACWY): 1 booster dose (on or after age 16)
