EOU-P0 - Personal Transportation

A personal Transportation Plan will be developed for students requiring modifications to transportation, which will address the specific physical, health and/or behavioural needs of the student.  The plan will be created with the school team, including the bus driver, resource teacher, and principal, with input from the Transportation Supervisor and the Student Services Administrator.

Refer to the Personal Transportation Plan template (EOU-E1).  This form will be completed and reviewed annually, along with the student’s IEP, with the school team as listed above.
