It is generally advised to healthcare professionals to consider following points for prevention of adverse drug reactions (ADRs); Enquire and document patient medication history for allergy or idiosyncrasy. The more medicines prescribed or taken, the more chances of drug interactions or ADRs. Age, hepatic, renal, or genetic factors may alter the drug disposition, thus more ADR chances. Clearly written prescription will not be misunderstood. IV admixture storage and administration should be as per protocols/literature. Special concern should be taken for new drugs in the market.
