Any probation data not provided in the webpages and dashboards linked above can be requested by completing this FORM.

All requests must complete the form or the request will be automatically declined. The form includes but is not limited to the following information:

• • Researcher(s) full name

  • University/company affiliation(s)

  • Research topic

  • Detailed description of proposed research (500 words max)

  • Data elements and years requesting

  • IRB approval (if applicable)
