Reminder: COVID-19 Vaccination Opportunities for Students Age 12 and Up

Dear PPS Families,


As we have shared with you previously, the Oregon Health Authority recently announced the approval of Pfizer’s two-dose COVID-19 vaccination for individuals age 12 and up. We are continuing to provide students and their families with vaccination support at the Oregon Convention Center (OCC).


Please visit the PPS Student & Family Vaccination website for information about our supported Wednesday vaccination opportunities and how you can request transportation to and from the OCC. This is the last week to access a first vaccine dose at the OCC. Second doses will be available until they close their doors on June 19.


Students age 15+ can sign up or walk up to the OCC to receive a vaccine. Students ages 12-14 must be accompanied by either a parent, a guardian, or someone designated by the parent. If accompanied by an adult other than a parent, a student must have a parent-signed consent form. Below is the consent form in all PPS-supported languages:



And just a reminder, vaccinations at the OCC and other locations are easy to access throughout the week. You can visit https://getvaccinated.oregon.gov/#/locator to search for vaccination opportunities in your neighborhood or check Multnomah County’s list of vaccination sites.

Age Guidelines

Students 15 and up do not need permission to get the vaccine.

Students 12-14 need one of two things to get the vaccine.

  1. A parent or guardian OR

  2. A designated adult from your parent AND a signed consent form

      • Here a consent form you can print and have parents sign: https://all4oregon.org/wp-content/uploads/2021/05/InformedConsent.pdf. We will be adding a translated versions of the form as soon as they are available from the health department.

      • Or, you can bring a written or typed note that includes the parent/guardian’s name, their relationship to the young adult, the young adult’s date of birth, a statement saying the parent/guardian consents to the young adult being vaccinated, and the parent/guardian’s signature.

Shuttle Service/Help with Transportation

Due to limited interest, we will be canceling the shuttle service in the weeks ahead. If you would like help with transportation, please fill out the survey above and inidcate you need help with transportation. Someone from our transportation office will contact your family.

Walk Up/Drive Up/Trimet

Students and families can also travel to the Convention Center on their own. There is free parking underneath the convention center and it is right off max and trolley lines. We will have PPS Tent outside the main entrance to help anyone with questions. The Convention Center is well staffed with a lot of support for families. Come stop by our tent for a granola bar and say hi!
