Individual Health Plans
If your child has healthcare needs that require nursing services while at school, an individualized health plan (IHP) will be developed for your child. This plan is updated every year. If your child carries an EpiPen or inhaler at school, a Medication Authorization Form must be completed and returned to the school nurse. Below are frequently used templates for common diagnoses.
A form like this is required from you, completed by your child's physician, if your child has asthma and needs to carry an inhaler at school. You will also need to submit the Authorization of Medical Release Form and the Medication Authorization Form to the school nurse.
