Vendor Registration

New for 2019-2020: Vendors must bill Portland Public Schools - Portland Partnership directly for students in the partnership taking classes in their program.

Below you will find the forms needed to be a vendor with the Portland Partnership for the 2019-2020 school year.

• A registration form needs to be filled out for all NEW vendors. If you are an existing vendor you will not need to fill out a new registration.
• A W9 tax form must be printed, signed, and returned to the school be scan or fax.
• All staff that work for the vendor, who will have contact with the students, must have a fingerprint background check completed. There are two forms. The LiveScan form you will need to take with you to have your fingerprints scanned. The release form you will need to sign and send to me. The best place to have this done is at any Michigan State Police post. Some county sheriffs offices will offer this service as well.
